CONTEST AND SWEEPSTAKES OFFICIAL RULES
NO PURCHASE NECESSARY DISCLAIMER
NO PURCHASE NECESSARY TO ENTER OR WIN. A PURCHASE OR FUNDRAISER BOOKING WILL NOT INCREASE YOUR CHANCES OF WINNING THE SWEEPSTAKES.
- SPONSOR
MOD Super Fast Pizza, LLC (“MOD Pizza” or “Sponsor”), 3245 146th Pl SE, Suite 100, Bellevue, WA 98007.
- PROMOTION PERIOD
The MOD Pizza Back-to-School Fundraiser Contest and Sweepstakes (“Promotion”) begins at 12:00 a.m. Pacific Time (“PT”) on August 24, 2026, and ends at 11:59 p.m. PT on November 15, 2026 (the “Promotion Period”). The Sponsor’s computer or timekeeping system is the official clock for this Promotion.
- ELIGIBILITY
The Promotion is open to authorized representatives of U.S.-based 501(c)(3) nonprofit organizations (“Organizations”) that are located within the service area of a participating MOD Pizza restaurant location (including company-owned and franchised locations). The authorized representative submitting the entry (“Entrant”) must be at least eighteen (18) years old and a legal resident of the United States at the time of entry.
The following are not eligible to participate:
(a) Employees, officers, and directors of Sponsor, its parent companies, subsidiaries, affiliates, franchisees, advertising and promotion agencies, and prize suppliers, and members of their immediate families (spouse, parent, child, sibling, and their respective spouses) or persons living in the same household as such individuals.
(b) Organizations in any jurisdiction where this Promotion is prohibited or restricted by law.
Sponsor reserves the right to verify eligibility at any time, including by requiring proof of 501(c)(3) status, authorized representative status, and geographic proximity to a participating MOD Pizza location.
- HOW TO ENTER
There are two (2) methods of entry:
(a) Fundraiser Entry. Book a fundraiser event through MOD Pizza’s community fundraiser program at https://www.modpizza.com/fundraisers/ during the Promotion Period. An Organization qualifies for entry if it (i) books a new fundraiser event during the Promotion Period, or (ii) has a previously scheduled fundraiser event that takes place during the Promotion Period. Each fundraiser event booked or occurring during the Promotion Period constitutes one (1) entry. Each entry will be ranked by its net eligible sales (as defined in Section 6) for purposes of the Contest Component and will receive one (1) chance in the random drawing for the Sweepstakes Component.
(b) Mail-In Entry (Free Alternative Method of Entry). To enter without booking a fundraiser, hand-print the Organization’s name, mailing address, phone number, email address, EIN, and the Entrant’s full name, title, and date of birth on a 3”x 5”card and mail it in a stamped envelope to: MOD Pizza Back-to-School Fundraiser Promotion, 3245 146th Pl SE, Suite 100, Bellevue, WA 98007. Mail-in entries must be postmarked by November 15, 2026, and received by November 22, 2026. Each mail-in entry will be assigned net eligible sales of $0.00 for purposes of the Contest Component and will receive one (1) chance in the random drawing for the Sweepstakes Component. Mail-in entries have the same odds of winning the Sweepstakes Component as fundraiser entries.
All entries become the property of Sponsor and will not be returned or acknowledged. Proof of submission is not proof of receipt by Sponsor.
- ENTRY LIMITATIONS
Each Organization may submit a maximum of three (3) entries during the Promotion Period, regardless of entry method. For fundraiser entries, events must be booked at least twenty-eight (28) days apart. For mail-in entries, each entry must be mailed separately.
Entries that exceed the stated maximum or that do not comply with these Official Rules will be void. Any attempt to obtain more than the permitted number of entries by using multiple identities, accounts, addresses, or any other method will void all entries submitted by that Organization.
- CONTEST COMPONENT: TOP THREE BY NET ELIGIBLE SALES
(a) Judging Criteria. Each Organization’s contest ranking will be determined by its highest single-event net eligible sales among all of that Organization’s entries during the Promotion Period. The three (3) Organizations with the highest single-event net eligible sales will each be awarded one (1) Contest Prize (defined in Section 8). Each Organization may win no more than one (1) Contest Prize, regardless of the number of entries submitted. If an Organization that would otherwise qualify for a Contest Prize has already been awarded a Contest Prize, the next highest-ranking Organization that has not yet received a Contest Prize will be selected. “Net eligible sales” meanstotal food and beverage sales generated at the MOD Pizza location during the Organization’s designated fundraiser event date(s), excluding tax, gift card purchases, and retail merchandise sales. Refunds, voids, and complimentary items are also excluded. Net eligible sales are calculated using the same methodology Sponsor uses to determine the 20% fundraiser donation. Sponsor’s records of net eligible sales are final and binding.(b) Tie-Breaking. In the event two or more entries have identical net eligible sales, the tie will be broken in favor of the entry whose fundraiser event occurred on the earliest calendar date during the Promotion Period. If a tie remains after applying this criterion, the tie will be broken in favor of the entry whose Organization first submitted its fundraiser booking or mail-in entry (determined by Sponsor’s records or postmark date).
- SWEEPSTAKES COMPONENT: RANDOM DRAWING
- Drawing. On or about November23, 2026, Sponsor will conduct a random drawing from among all eligible entries received during the Promotion Period (including both fundraiser entries and mail-in entries). One (1) entry will be selected to receive the Sweepstakes Prize (defined in Section 8). The drawing will be conducted by Sponsor or its designee, whose decisions are final and binding. Entries that won a Contest Prize are eligible for the Sweepstakes drawing.
- Odds. The odds of winning the Sweepstakes Prize depend on the total number of eligible entries received during the Promotion Period.
- PRIZES
The following prizes will be awarded:
| Prize | Selection Method | Number of Winners | Value per Winner | Total Value |
| Contest Prize | Top 3 by net eligible sales | 3 | $1,000 | $3,000 |
| Sweepstakes Prize | Random drawing | 1 | $1,000 | $1,000 |
Total number of winners: four (4). Total approximate retail value of all prizes: $4,000.
Each prize will be awarded in the form of a check made payable to the winning Organization. Contest Prizes will be sent with the Organization’s standard fundraiser donation check. The Sweepstakes Prize will be processed separately if the winning entry is a mail-in entry. All prizes will be processed after the Promotion concludes and take approximately four to six (4-6) weeks to process. Sponsor anticipates that all prize checks will be received by winners no later than December 31, 2026.
Prizes are non-transferable and may not be substituted or redeemed for cash, except at Sponsor’s sole discretion. Sponsor reserves the right to substitute a prize of equal or greater value if the advertised prize becomes unavailable. No more than four (4) prizes will be awarded. Each Organization may win no more than one (1) Contest Prize. An Organization that wins a Contest Prize remains eligible for the Sweepstakes Prize.
- WINNER NOTIFICATION AND VERIFICATION
Potential winners will be notified by email at the address provided in the entry within five (5) business days of winner determination. Winning Organizations will also be announced on MOD Pizza’s fundraising homepage at https://www.modpizza.com/fundraisers/.
Each potential winner must respond to the notification within seven (7) calendar days confirming acceptance and providing any requested verification, including proof of 501(c)(3) status, authorized representative identity, and a completed IRS Form W-9. Each potential winner (or authorized representative) may also be required to execute and return an Affidavit of Eligibility, Liability Release, and Publicity Release within the same seven (7) day period.
Failure to respond, failure to provide required documentation, or return of any notification as undeliverable will result in forfeiture of the prize, and an alternate winner may be selected (by net eligible sales ranking for Contest Prizes, or by random drawing for the Sweepstakes Prize).
- TAX RESPONSIBILITY
All federal, state, and local taxes on prizes are the sole responsibility of the winner. Each winning Organization will receive an IRS Form 1099-MISC (or other applicable tax form) for each prize valued at $600 or more. Winners may be required to provide a completed IRS Form W-9 or equivalent documentation before prizes are awarded.
